DISCLOSURE REGARDING CONSUMER REPORT AND/OR INVESTIGATIVE REPORT

Fort Wilderness Ministries may obtain information about you for employment purposes from a third party consumer reporting agency. As such, you may be the subject of a “consumer report” and/or an “investigative consumer report” which may include information about your character, general reputation, personal characteristics, and/or mode of living, and which can involve personal interviews with sources such as your neighbors, friends, supervisors, or associates. These reports may contain information regarding your criminal history, social security verification, motor vehicle records (“driving records”), verification of your education or employment history, or other background checks. Further, you understand that information may be requested from various Federal, State, County and other agencies that maintain records concerning your past activities relating to your driving, criminal, civil, education, and other experiences.

You have the right, upon written request made within a reasonable period of time after receipt of this notice, to request whether a consumer report has been conducted about you, disclosure of the nature and scope of any investigative consumer report, and to request a copy of your report. Please be advised that the nature and scope of the most common form of investigative consumer report obtained with regard to applicants for employment is an investigation into your employment and/or education history. The scope of this notice and authorization is all-encompassing, however, allowing Fort Wilderness Ministries to obtain consumer reports and investigative consumer reports now and throughout the course of your employment to the extent permitted by law, unless you otherwise revoke your consent by providing written notification to Fort Wilderness Ministries. Fort Wilderness Ministries (FWM) is an independent faith mission organized for the purpose of promoting the Gospel of our Lord and Savior Jesus Christ and serving the local church through various means.

4. Providing adventure programming to give campers new and different experiences
to strengthen their relationship with Jesus Christ and with fellow campers
(1 Corinthians 9:22-23).
5. Sharing the Gospel of redemption through the death, burial and resurrection of
Jesus Christ (Romans 1:16, 1 Corinthians 15:1-4).
6. Teaching Biblical worldview principles to children and adults alike in a way that helps them understand and defend their faith at home, school, and work (Colossians 2:6-8).
7. Using every opportunity to build a relationship with churches, campers, community,
and staff that allow for spiritual life changing conversations and decisions
(Ephesians 4:12-13).

As an employee at FWM, I recognize and understand Fort Wilderness is a religious, non-profit Christian ministry representing Jesus Christ in all aspects of our professional and personal life. Fort Wilderness requires its employees to live their lives as Christian role models according to biblical precepts (Rom. 10:9-10; 1 Tim. 4:12; Luke 6:40). I agree to conduct myself in a way that will not raise questions regarding my Christian testimony. I understand I am expected to demonstrate a teachable spirit, an ability to share love for others, a willingness to live contentedly under authority and a commitment to follow the Matthew 18 principle when an issue arises with fellow employees, volunteers and/or ministry leadership.
I understand and accept FWM expects its employees to maintain a lifestyle based on biblical standards of moral conduct. I further understand moral misconduct, which violates the bona fide occupational qualification to be Christian role model, includes, the values in FWM Statement of Faith and Values. Further, I have read, agree with and agree to abide by the religious standards set out in FWM Statement of Faith and Values. As an employee of Fort Wilderness, I will maintain a lifestyle based on biblical standards of conduct. I recognize failure to do so may result in a reprimand, or, in some cases, dismissal from employment.
